2004 ford freestar van idles rough?

check engine light with codes P2195 and P2197 (O2 sensors) engine too lean (getting too much fuel) what do I do? cleaned fuel injectors, used gas line antifreeze (it was -40 outside), should I change the spark plugs?

Also had the P0171 and P0174 lean system... it looks like problem could be vacuum/pcv hose?
